Hello Everyone, from today (30/09/2019) you should receive a letter from us in the post regarding GDPR and the storage of the information we currently hold on you. Due to the change in law- we now need your written permission to store any old information we have on you and your family.

If these letters are not signed and returned, all of the information we have on you will have to be destroyed and you will be removed from our database. If you would like to stay on our system, and continue to access our services, these letters must be returned before 31st October 2019. After this date, if you have not returned this form, you will have to go through the assessment and referral process again to access our support in the future. If you no longer wish to receive support from us, simply ignore the letter and do not return it. We thank everyone for being a part of our project!

Please let us know if you haven’t received the letter by the end of the week (Friday 6th October 2019) via Facebook, email (levi@styoungcarers.org.uk) or give us a call on 0191 427 2795. This form is attached below if you would like to print it off yourself. If you need anything clarifying just ask.
